Before starting this form, please be sure you have at the ready the following items needed for upload during the application process:
- A digital file (.xls, .PDF, .rtf, .doc, etc.) which contains a brief history of your firm, as well as a brief description of the layout of your facility
- All names of proposed faculty or staff from the builder’s firm who will have contact with participants (giving instrument demonstrations, leading classes, chaperoning on POE-Tech field trips, etc.)
- A digital file (.PDF, .rtf, .doc, etc.) listing all proposed instruments and sites for lessons, workshop purposes, or concerts
- A digital file which is a scan of the “Statement of Approval” signed by the dean of the chapter. (Form is found on the third page of Section 6.1.2 of the POE Handbook.)
Please note: The usual deadline for application to host a POE event is April 1, in the calendar year prior to the year in which an event will take place. For example, the chapter application to host a 2025 POE event must be received by April 1, 2024.
