Before starting this form, please be sure you have at the ready the following items needed for upload during the application process:
- A digital file of the chapter’s complete financial statement (e.g., balance sheet and income/expense report) for the most recently completed year
- All names of proposed faculty
- A digital file (.PDF, .rtf, .doc, etc.) listing all proposed instruments and sites for teaching, practice, or recitals.
Please note: The deadline for application to host a POE event is April 1, in the calendar year prior to the year in which an event will take place. For example, the chapter application to host a 2026 POE event must be received by April 1, 2025.
