Fellowship Certification Exam Application Form

Fellowship Examination Application for June 4 & 5, 2026

To apply for the Fellowship (FAGO) exam, please complete this application form. The final deadline for submissions is April 1, 2026. All fields marked with a red asterisk are required to successfully submit your application. For comprehensive information on exam content, study materials, and preparation strategies, please visit agohq.org/certification.

Please note that no honorific titles (i.e., Dr., Prof., etc.) or academic suffixes (DMA, Esq., etc.) are utilized on AGO certification documents.

Section 2 of upper level exams may be administered at any chapter. However it is recommended that you contact the chapter dean or education coordinator to confirm that the chapter knows you intend to take Section 2 of this exam there.

Performance from digital scores on electronic devices*
If my performance is from a tablet or other digital device using electronically/digitally stored and accessed scores, the scanned copy is my legally owned or downloaded copy, which includes 3 possible scenarios:

1. A scan made from the candidate's (my) own legally owned (i.e., my own copy) physical paper score.

2. A digital score purchased by the candidate (me) from a publisher.

3. A downloaded IMSLP score from the specifically approved list of IMSLP scores in the Certification requirements.

I also certify that I will NOT be using another person’s scanned copy for performance by the candidate (me).

[Through a cooperative arrangement, members of Music Teachers National Association are eligible to take AGO exams at the AGO member rate and AGO members are eligible to apply for MTNA Professional Certification at MTNA member rates.]
Your AGO member ID is a 4, 5, or 6-digit number located to the upper right of your name just to the left of the membership expiration date on address label of your TAO magazine. Please call AGO Headquarters at 212-870-2310 for assistance if you don't know or can't find your member ID. MTNA members who don't know their member ID number should contact MTNA at (513) 421-1420 or by email to: mtnanet@mtna.org.
